It doesn't have to be anything fancy. You can just make a sheet with two columns: one for the date and one for the time in seconds. Title the page with the skill being taught/learned (such as "Multiples of 5 and 6"). You may want to use a half, or even a fourth, sheet of paper for each set of problems in order to save paper (that should be plenty of room, anyway). Let me know if you need me to clarify anything.
