Conference Sheet

This year we are required to have our students come to conferences with their parents. We need to have them set goals. I am wondering if anyone has a form that they use for planning for this that they would be willing to share? I would like a form that either the student can fill out or one that I can do as preparation for the conference so the student and I have something to guide the conference.
