I used the same kind of method. I used a clipboard. The students were listed and I had my rules numbered. When the student broke a rule, I simply put the rule number broken next to his name under that day of the week. I love it because it really backed me up in parent conferences. I could tell a parent exactly which day a student missed an assignment or misbehaved. It also helped me keep up with attendance at a glance.
One more way it helped me. If my students went to art, music, computer lab, my clipboard went too. The other teachers were fully aware of the system we used. When I picked them up, I looked at the clipboard and knew immediatly what and who the problems were.
