I see the same behaviors. This is what I have done.
I proceed with the class as normal, making sure to casually keep an eye on the offender but not make eye contact. As long as some part of their body stays on the chair and they are not in danger of hurting themselves I don't talk to them. I make sure to praise the obedient children, pointing out their good behavior. I make sure to include a short really fun activity , song, story etc that the offending child usually enjoys particpating in. I do not acknowledge the offending childs raised hand or voice, for at least five minute (you have to judge this yourself) or until I can see some change in attitude. I was amazed at how well this has worked, I have had to use it several times this week, but each time was shorter and shorter. I used it with several other challenging chidren and WOW did they get the point fast. No anger, no long speeches, just "stand up!" take the chair "here is where you sit , because that place over there is for obedient children only" and back to your regular class. hope it works for you. One thing this boy is craving is attention, but I refuse to attend to his bad behavior. let me know how it goes.
