Shouldn't she know already from her past evaluations that her performance is poor? Just write her strengths first, outgoing, warm, etc, then say needs improvement in Behavior managment. Be truthful please. If she is not a good teacher then her reference should refer to that. Say she might be better suited for highschool. Some people just don't have the knack or instincts needed for elementary. You could say that she needs for mentoring or monitoring. Shouldn't you have discussed this with her advisor and the student teacher in a meeting. THis should not be a surprise to both of them.
