I had my class write a letter to one of the characters and we talked about things to say to people who have experienced a loss.
After chapter two i had them write a newspaper article on the accident.
I also had them reflect after each chapter and make predictions about the next...and we did a lot of talking about tough topics and how the book made us feel.
I also had them create masks from paper mache. On the outside they painted how the world sees them...on the inside they painted how they feel when they are sad..or wrote words that expressed feelnigs when they were sad. (like when they lost a pet, or loved one..or just sad)
I have other ideas too...let me know if you need more!
