I imagine you've tried a whole lot of things already, and you probably tried this, too, but I had a student like him once, and this worked for me. When he started crying and whining about how I wasn't helping him, I told
him that he was wasting my time and I was timing him. Whatever time he wasted, he would make up during recess, gym, or computer class. I told him that I was not going to do his work for him and when he could start acting like a 4-th grader I would be willing to help him like the other 4th graders. I would have him stay in at recess or sit out in the hallway during gym or comp. He could
not join the rest of the class until he finished what he should have finished during the time he was whining. I would just ignore him when he said things like, "You aren't helping me, I hate you, I am stupid," etc. After about 20 minutes, he would realize how much he was missing and ask me a specific question:"When you say to shorten this, what does that mean?" That's when I would start helping him - but only if he was cooperating. If he wasn't, then I would start ignoring him again. I wasn't in that class for long, but after while, his tantrums became shorter, although didn't disappear.
