For b and d I post 2 charts with the letters formed this way:
for b: the stick is a bat, the ball is a baseball
for d: the ball is a drum, the stick is a drumstick

using hands (left first) make a bat, then add a balled up right fist, saying "bat then the ball"

for d, using hands (left first) ball up the left fist then add a right hand stick, saying "drum then the drumstick"
As they add the ball or stick, they see the formation with their hands.
It makes a great chant: "bat then the ball, drum then the drumstick, bat then the ball, drum then the drumstick" (we use a drum) and the kids use their hand signals when writing. I find this to be the most effective technique yet.
