I teach second grade. We have bathroom breaks at recess (both morning and at lunch) and a break when we go to P.E. in the afternoon. It sounds pretty comparable to your times. At the beginning of the year I had one student in particular that asked to go to the bathroom during class frequently. I talked to her parents about it to make sure there wasn't a medical condition involved and once I got that cleared we set up a policy for going during class. If a student needed to go during class time, they lost 5 minutes of recess. This prevented them from going just to go. It worked great! I personally don't think it's too much to ask them to only go during the provided times.
