We use a calendar system. Each student has a calendar in which they have at all times. If they miss behave, or do not have their supplies or materials they get a check on that particular date on the calendar.

We use
the back of the calendar for missing assignments. This allows us, the student, and the parent a reference to look at for all missing assignments. (Great record keeping device)

At the end of each month, we come up with a number for example: 4. (This number is never revealed until the last day of the month, so a child doesn't give up half way through.) If a child has 4 marks on the front

and back of their calendar they get to go to the reward room. Which includes a fun activity. Students that have more than 4 checks go to the opportunity room. Where they sit and do missing assignments, write sentences, and other horrible busy work. We try to make this room miserable so the students work harder to get into the reward room next month. They also learn goal setting, and the consequences to their behavior.

This works well because we have 3 teachers in 5th grade and rotate. If there is only you. You can just provide a reward at the end of the month for those who have earned it. (Example: extra recess, candy, homework pass. Whatever motivates your students).
