I had this problem with one of my fifth grade classes and talked to the kids about it. They came up with a solution that I have used for several years now-put a limit on the 'emergency' bathroom trips. Talk to them about what a real emergency is and how often one may come up. They put a limit on one emergency bathroom trip every week. It is easy to keep track of if you use a sign out sheet.
Some teachers have used egg timers near where they keep the hall passes to limit the amount of time they spend in there. Hope this helps!
