I had this problem last year as well.
I did a notebook much like the poster above.
A lot of my students would get the sheet and then never put it in the notebook. If I seen a problem we would discuss it as a class, not calling down a paticular student. Also, if I seen a student mentioned several times by several students we would conference quietly.
