My first thought was abuse when I read this. You say the family is dysfunctional, do they give him enough attention? Make him feel important, etc.? Could he be doing this for attention? I know you said that the nurse, SPED,
and administration are aware of this, but is there a possibility that you, the administration, the nurse, the SPED, the boy, his parents, and the guidance counselor come together to have a meeting? You could meet and discuss what is going on, perhaps come up with a plan. do you have access to this boy's school record? You could look in their and see if it mentions anything in regards
to this problem. You could also talk to his teacher's from previous years and see how they handled the situation. Another possibility may be that he has been made fun of in the bathroom by older kids? Perhaps for pooping and that's why he doesn't want to go in there anymore.
