The teacher next door had one of these last year. For this child (a third grader who had done this since Kg,) it was a control issue. He would urinate all over himself when he got mad at the teacher, at other students, whatever. He once defecated in his pants in the lunchroom and slid up and down the bench leaving smear marks. The parent insisted there was no emotional problems (although that was obviously not true.) The teacher suspected abuse, but the parents conveniently ignored that idea. There was nothing the teacher could really do for the child since his parents didn't want to help (didn't acknowledge a problem) but send him up to the clinic to change when it occured (at least once a day!)
