Fluency question

When you are determining a rate for fluency, are you using a text the student a)has never seen, b) has been able to preview or c)has practiced many times?
Also, our district uses Open Court. The teacher manual guidelines state 82 words per minute in beginning 2nd grade and 124 by the end of 2nd grade. (they are graduates of the speed reading course!!!) I think 90 wpm by the end of 2nd grade is much more attainable.
I still question the familiarity of text for fluency evaluation....
