I think it is great that you are being honest when filling out the Dr forms - the Dr needs to know exactly what is occurring in school. As for modicifcations, I don't think you are legally responsible for making modifications/accomodations if they are not written. However, I would suggest that if you do choose to try a few (to see what works and what doesn't), be sure to write everything down for documentation. This would include what you tried, how long you tried it (3 weeks, on-going, etc), and the results. That way, when you do meet for a conference to discuss the student, you can show the need for accommodations. Good luck!
