First of all, you are feeling down after having such fun last year being a teacher. This is normal.

Teach to the students'level; you are doing the students and yourself wrong if you try to teach above what they are capable
of doing. If you have low and high in the same group, modify for the lower kids (ask lower grade teachers if you can look at their materials and even borrow to copy some materials).Group into smaller subgroups. Its hard work, but it will work. Give them the phonics, if any questions are asked, just be plain spoken and say, "You/ Your child is very behind in their grade level work, and
what I am doing is to help bring you/ your child up to grade level. It won't happen this year, but it will get them closer to where they need to be. Will you help me, by making sure homework is done and reading and studying with your child?" If ask how this could have happened just reply "I don't know." Also, you know you did everything possible and will feel no regrets at the end of the year.

But first of all, get a discipline plan in order and carry through with it!! Get the unrulys out or under control. If you work with other teachers tell them Johnny needs to sit at recess and do his homework from yesterday. If they don't comply, then put Johnny in a corner until he wants to be a polite student, and phone his parents to let them know what has happened. Keep him into the next period until he complies with your standards. Document document document !!!!! Proof Proof Proof!!!

You can teach the same basics to the whole class, just modify for the lower students and challenge the higher students.

Those students who are doing correctly, give them meaningful projects, even some that get them in the media center while you work with "the Others".

My heart goes out to you. You cover your assets, and you do your best. Let the parents worry about why little Johnny/Sally is behind.
