behavior chart

Our school has a section on the report card about personal development. We decided (as a school) to us these as our behavior chart. We give grades (S-N-U)for: works independently, completes work, follows rules... So we have a chart with a pocket for each student and when they break a rule they pull a ticket corresponding with the problem. This way we have documentation for our "N" on our report card. I also have a daily behavior sheeet that documents this to parents and they sign it nightly.
