A teacher I know, more organized than me, glued library pockets in all her books. You can buy library pockets in big quantities. Then, she wrote the title of the book on an index card and placed it inside the pocket. The kids would take out the index card and write their name and date on a line and give the index card to her. She would file it away in her index card box as a reminder that they had her book.

Another simpler idea is to type up a book check-out/return form. Title of book, child's name, date checked out, date checked in, etc. The kids would sign out the books for a 2 week period. A classroom helper can keep track of the library form. I usually have a few responsible students that enjoy having this job.
