| I use a very simple method- I just laminated a large poster board and went and bought clear plastic disk holders that have adhesive on the back and stuck them to the poster board and number the pockets (each student is assigned a number at the beginning of the year that they use to line up, put on their papers, etc). I then have four colored cards that I keep in a basket near the pockets. If the student has perfect behavior, then their pocket stays empty, fist warning they have to pull a orange card (5 minutes off recess), second offense...yellow card (10 minutes off recess and a note home), third offense is a blue card (no recess and note/call home), and last offense is a purple card (sent to office). Each day is a fresh start and everyone starts off with no cards in their pockets. I had my class trained to watch for the signals to pull a card (a stern look and a nod of the head toward the cards or pointing to the cards) so that I wouldn't have to stop teaching or have to call anyone out in front of the group and embarass them. Last year, none of my students got past the third card- so I think that it was effective for me in my room, but again, it's not really the card ststem that encourages the students to behave- you have to!
