M. Clay's book is called 'An Observation Survey' and has 6 subtasks... Letter Identification, Word Test, Concepts About Print, Writing Vocabulary, Writing Dictation and a Running Record of leveled texts. Excellent resource. It's published by Heinemann. I'd also suggest the Developmental Reading Assessment by J. Beaver published by Pearson Learning group. Post or email if you need additional assessment info. ;-D
