Not rehiring

How many of you work in a school that has not rehired someone? I honestly thought that was a rare occasion, but I heard our school is doing that with someone (not me thankfully). I haven't heard exactly why, but it doesn't seem to be anything "major" - just not doing a good job with their teaching and after constructive criticism was given they still didn't improve. They're a first year teacher. Just wondering how often that happens.
