The Guided Reading books by Irene C. Fountas and Gay Su Pinnell are great. I own the one titled: Guiding Readers and Writers- Grades 3-6.
They also have a book for grades K-2. It takes you step by step on how to set up a guided reading program, with lists of leveled books in the back.

Try typing Fountas and Pinnell into a search engine, I'm sure you will come up with some good web pages.
