fluency

This may sound very simple, but I'm finding that it's working with my students. First, I make sure they are reading a book at or near their level. Then I model good reading for them. We've actually discussed what fluency means, and I model good fluency for them, and point it out when I'm doing it. We also talk about "boring reading", enunciating our words, and reading for an audience.

I know it seems vague and simple, but these things have worked in my classroom. Oh, and they need to read, read, read as much as possible.
