I don't know if this will help, but I just recently found my internship evaluation from a few years ago. My host teacher had told me that I needed to not be so set in my ways (stubborn) that I didn't take/try advice from others. She worded it in my write-up like this, "So and So has recognized the need to moderate her self confidence in order to draw upon the experience of other teachers." I thought that was a very positive way to say, "You don't know it all! ")
You may also want to bring up the need to re-evaluate a behavior plan when it is not totally effective. Hope this helps!
