On the childrens desk is a pocket envelope (the the ones in books in the library. Inside are cards (cut out of construction paper and laminated) green, yellow, orange and red. They all begin on green every week.
green-making a terrific choice!

Orange=1st strike, warning

Yellow= 2nd strike, lose Friday Brain quest and 10min off of center time

Red= 3rd strike, 15 min of recess, Brainquest, and center time and they get a phone call home

I have them change the card from color to color. All I do is say "so so 1st strike"
Those who stay on green daily receive "cash" which they can use to buy different things off of a menu.

I rarely have children on red.
