I'm not surprised by the lack of response to your question. Most of the surveys I have seen were geared toward older kids. I ended up making one of my own. It was okay but as expected it needed some tweeking. Here's the general
idea. I placed icons in a row and boxed off the section. The children had to choose which thing they would rather do and circle it. I made several of these boxes with icons for multiple intelligences, maybe four choices in a box. The icons overlapped different areas. Playing with blocks could be tactile and logical mathmatical, etc. Don't include any TV icons. They won almost everytime
and skewed things. You will find your results are only estimates of their interest but you'll get needed information out of it. You can also point out the problems with your survey in your evaluation. Remember you are learning and being able to recognize problems on your own points to your intelligence.
