Lesson planning sheet

Hi, I use a lesson plan sheet that includes a place for each student's name (6 at most) and a space after it to take notes. That takes up about the first half of the page. Then I have 6 boxes below. Box 1 has the lesson focus.
Box 2 has the the standard that this lesson teaches to. Box 3 has the points I will use in my book introduction and an introduction to the strategy we will be working on. Box 4 is a place for the points we will discuss after the reading. Box 5 is a place to put other points that have come up during the lesson. And box 6 is a place to remind me to do closure and remind the students
to practice this strategy on their own. At the top next to the student's name I write notes and observations that I see while I listen to each student read. We use running records, RESULTS, and DRA for assessments. We use this lesson plan sheet for grades K-5 and our school is Title 1.
