I teach 2nd and use a book called Skills for Young Writers (Instructional Fair, TS Denison). I believe that they have a book for each grade level 1-6. I like it because they list the skills that the paragraph touches on at the top of the page, telling the students what to look for. At the bottom of the page it gives suggestions for revisions, kind of an extension activity.
