It sounds like you have some valid concerns. I am assuming that you are in a public school. It is the regular teacher who has to speak to the guidance counselor about concerns and perhaps have the child referred for testing. This takes certain paperwork and can be lengthy. The parents would be invited to an official meeting and would have to give their permission to test the child. It is always very difficult at such a young age, but this is the time to be proactive.
