Things to perhaps think about just in case it happens again.

1) If the girl is shy she may not feel comfortable drawing attention to herself. If this is the case perhaps you could work out a signal she could use if she needs to use the bathroom.

2) Is there a problem with the bathroom itself. If it's a bathroom with stalls, perhaps she has been teased by other girls looking over the stall or under it. Or she just may not be comfortable using the bathroom if anyone else is in the room.

3) A medical condition.
