anecdotal notes

Ihave been teaching in a High/Scope school for 6 years,and have tried many ways to get documentation organized. My two favorite ways are as follows: 1.Create a form using your word processor. At the top of the page type
Name__________. On the rest of the page, I make a chart with 2 rows and 3 columns. This gives you 6 spaces per page. Each child has their own sheet in this 3ring binder. When I want to write a note, I jot it on whateverishandy, and then transfer it onto the child's sheet. I am always able to quickly see who I need to get more notes on. My assistant and I split up the work. She is
responsible for her smallgroup, and I for mine.

the second way is taken from notes home. I purchased some of those carbonless notes home thru reallygoodstuff.com. These have been invaluable with documentation. I can write an anecdotal note home if a child did something particularly interesting at school that day (or particularly terrible!) I send the white copy home and put the yellow copy in the child's portfolio. This is great because the parent gets an idea of how the child is learning at school within our framework, and I have a copy that I didnt have to leave the classroom for. Hope I was some help!
