does he have a current IEP to work with? you are currently the least restrictive environment and I'd say that judging by his behaviors...he is doing his best to tell you that he is NOT happy and that this placement
is not comfortable for him.

I'm assuming here that you've gone thru all the channels, spoke to parents, sped teacher yes? well if not do so please, they're your best resource...if yes then I'd call a new IEP if I were you because this child in his own way is CRYING for help...his behaviors are telling you that he can't handle your classroom and needs either some/more pull out time

or maybe another placement altogether such as a self-contained room.

and speaking of parents have you asked what type of medication he is on and when he had his last med check? there are medications that help to control those noises. It has been my experience that the more/greater the noise the higher the anxiety of the child.

12 is quite old for 4th grade so you could also argue age appropriate classroom...because most 4th graders are 9 or 10...12 is puberty, etc...a whole new bag of worms.

as for the teacher that was injured, why was she holding on to the child? Usually with a child of this type you should not hold without telling the child you're going to touch him first...he may be tactially defensive to touch especially a strange teacher.
