I have a really good information sheet for parents to fill out. It asks if the child has any siblings and ages, and who lives in the same house with them: mother father stepfather stepmother. That will give you some insight
on their family life. Also it asks for any religious traditions I should know about, and what holidays might not be celebrated in their religion. It asks if the child wears glasses and if they should be worn for reading only or at all times. If he/she has any specific health issues I should know about. Then the best part at the bottom it asks the parent to share any pertinent information
about their child that they think I should know. I get MORE out of that one statement than the rest. Parents can really give you some insight on a child. Some would never tell you those things face to face, but have no trouble doing it in writing.
As for the wish list: I ask for disposable cameras, envelopes, snack size-quart size- and gallon size ziplok bags, paper plates, paper cups, plastic silverware, napkins, erasable pens, pencils, erasers, markers, puzzles, to name a few. I make a tree on my board and write the items on a construction paper apple. I ask parents to please take as many as they wish to help out. Usually everyone takes 2 or 3 and I get a lot of supplies. If I think I need two boxes of envelopes, I would put two apples up with that on it. I call it my GIVING TREE. You'll be pleasantly surprised by the response I think.
