Last year I subscribed to edhelper.com. It offers what you are looking for at every grade level. (I teach special ed. and it was a big help last year. I was able to create worksheets for each individual student.)
It also offers worksheets for just about every math concept we have to teach, not just word problems. It allows you to create worksheets related to the seasons and themes, you can create word puzzles where the students put their answers in a crossword puzzle, it even creates worksheets using student's names if you want to. You can create as many as you like and ANSWERS ARE INCLUDED.
I love the site and will subscribe again this year. I is a great value. I believe it is $19.99/year.
