We have to send out something at Interims as well as 9 wk. progress reports. We have a stick label with the students' name on it that is attached to the reg. report card and list the area, for example: John Doe
This students grade in Language arts reflects assistance by Resource: Progress shown __, Needs Improvement___ and it is checked. For their regular report card, we result their IEP goals with a date stating how and if they are progressing. This is all backed up by using Probes which I keep in a file to show if there are any questions. Maybe this gives you some ideas.
