I use the book Teachers' Messages For Report Cards" by Marie McDonald. In fact there are several teachers in my school who do the same. I have never used one verbatim but it has helped get my thoughts together and get more focused so I can say what I have to say in the small space that I have to write in. The book was $7.99 and well worth it. I keep it in my report card file. When I get stumped, I can drag it out.
It has a complete section on weak, slow, and immature work as well as the slow child. You could probably find something appropriae in one of those sections. I would try to find something positive he can do and note that along with his struggling with the academics. Good luck.
