We use STAR for reading and math assessment. It's a computer program, if you're not familiar with it. It shows the reading/math level of the students. Then we ability group them for reading, based on the test and the teacher's recommendation. We do an hour of "Power hour" each morning with ability grouped reading. During that time frame we do novel/author studies, literature circles, vocabulary, etc... - based on the needs of our students. We also have an additional hour of reading in our regular classroom. That time is usually spent with a basal and science and social studies material to supplement those subjects. Our students just scored very high on their reading, so I feel confident it seems to be working. I'm always up for new ideas, though!
