The Alberta Diagnostic Reading Test

They also have one for math.

I do not have the publisher information handy but I know it was developed by Alberta Learning.

It's not a standardized test either - more of a diagnostic tool.

I'll be back in school next week so if you can't find the information before then please feel free to email me and I can look on my copies for any other information.
