"the incident"

If it were me, I would simply tell her that this is the first time this has happened and that I want to simply "make her aware" of the incident for a couple of reasons. First of all, to see if this is an isolated incident (perhaps it happens at home due to bad aim or other reason). Secondly, to let her know that accidents and "distractions" can happen, but that I believe in personal responsibility (even at 4 years old). That is how we all learn responsibility. We have to be responsible for our own actions regardless of accident or "distraction". That sometimes involves cleaning up our own messes. That is how we learn to be responsible citizens.
