Of course I don't know this child or all the circumstances but I have just a thought. I had a child last year that missed recess nearly everyday for misbehavior in the classroom. I also use the stoplight method and we also have 15 min. to play. I felt like it was a suitable punishment but it was absolutely not effective. So one day, I had the student come talk to me during recess and I discovered that he did not like recess because he was not the active type. So, from then on instead of having him sit out at recess, he had to walk the perimeter of the playground to owe his time. He HATED that and soon his behavior began to improve. I don't know if this will help you but maybe it's something to think about.
