I would send the child to the school nurse and have him looked at. The school nurse could send a note home concerning the possible health concerns stemming from the odor. If it turns out the child's breath is just bad, I'd have a special toothbrush and toothpaste that I'd keep in the room just for him. I would also keep mints on hand and offer one to him when needed. I would talk to the parents about what kinds of breakfasts foods might help him and explain to the parents what you like about the kid and how you don't want him to be embarrassed. and see what they come up with.
