You are right. You just need to model and practice. I would enlist the help of a responsible student. Tell them that the student and you are going to act out for the class the incorrect way to act when asked to take out
your book, or while papers are passed out. Then act out the appropriate way. Then have everyone try it.

You could write the word "RECESS" on the board each day(you can write it once for the whole class, or if your students sit in groups, write it in different colors for each group). Then, each time you have to remind them to be quiet erase a letter. For each letter erased, you deduct

a set amount of time for recess. If all letters get erased, then sorry, no recess! This will get students involved in making sure everyone is doing what they are suppose to do because the punishment involves everyone. I think this might work for your situation. I hope so!
