My students use the restroom before coming into class in the morning and also after lunch. After lunch, it works like this: We all come in and read silently. I give the girls pass to a girl and the boys pass to a boy and they each go to the restroom. When they return, they hand the pass to the next person at their table to go. The pass eventually makes it all the way around the room. It works very well, and keeps me from calling out their names when it is their turn to go.
