Hi! I got this idea from my job-share partner, and it works really well. We have a D-Slip (detention slip) chart, it is on tag board and we have those library envelopes glued on. We write each students name on an envelope.
On the bottom, I have 1st warning, 2nd 10 minute time out, 3rd lose a recess and note home, 4th talk with parents and principal. In the envelopes I have pieces of paper in different colors (I use yellow, red, and blue) When a student misbehaves we simply say D-slip to them, and they get up and grab the appropriate color of slip and fill it out with their name, date, offence, and
what they plan to do different, and return it to their envelope. I collect these each night, and file them for conferences.
I was tired of students receiving additional attention by getting up in the middle of class and getting a slip then returning to their desk to fill it out and then walking back to the chart to put it in their slot then back to their desk (all during class time) so I am trying something different this year. I colored popcycle sticks with green, yellow, red, and blue ends and put them in each students slot (green facing out). If I am teaching a lesson and a student is misbehaving I simply walk to the chart and turn them from green to yellow, and don't say a word. That student then stays in a few minutes from recess to fill out a D-Slip. This actually seems to work better, and not be so disruptive, though I still send them to the chart during their work time.

Hope this isn't too confusing, I have screaming toddlers at home! :o)
