Our school puts three plastic cups on each table in the lunchroom (blue, yellow, and red). The red cup is on the bottom turned upside down, then the yellow on top of that, and the blue is on the very top. If the kids are loud or unruly at the table then they get a warning. If a second warning is given they change the cup to yellow. If a third warning is given it's on red. If they get too many red cups they go on silent lunch.

When the teachers come to pick the kids up after lunch to take them to recess the kids let you know what color cup they got. Teachers reward for the blue cups and consequences are faced for a yellow or red cup. Maybe this isn't an original idea, but it seems to work in our school.
