You might want to get BASIC READING INVENTORY by Jerry Johns. It is a book full of info. Included are leveled passages for running records, the forms to record on, as well as other kinds of reading assessment. It is very user-friendly, in fact, you could pick it up without having ever done any reading assessment before and do an excellent job! This book can be ordered through Barnes and Noble. I went to the University where Johns is based and tons of professors there use this as a text in reading methods!
