I have a worksheet that has a bunch of grapes on it and says we missed you a bunch, on each grape their is the title of a subject. You could make your own or something similar. I write the assignments on each grape and then attach any worsheets to this title page. I also encourage parents during the first weeks of school to call in the morning if their child is sick so I can prepare their missing work and then they can pick it up by 2pm. This works well and doesn't put the children so far behind when they come back.

I have also seen teachers make this a students job, and they collect an extra of each assignment and put in in a folder ready for the sick child the next day.
