There is another notebook online that looks good also. I just found MOOSE first and started using it. The other notebook is called OTTER. "Organizational Tools Teach Everyone Responsibility" You might take a look at that and adapt what you think you need in your situation.
I understand that some teachers charge their parents for these notebooks (as much as $6.00) but although they are expensive to make, in both time and materials, I think they workout over the long run...I usually take my expenses out of pocket. Our school furnishes most first grade supplies and charges only a $5.00 supply fee and it is hard enough to collect that from some. I do tell my parents that they must either replace the notebook them selves or pay $5 for a lost or damaged notebook (beyond repair and using).
Some info says use wirebound notebooks for Parent Teacher Communication. I did that the first year..some parents tore pages out to give paper to older siblings, smaller siblings drew on it etc. This year I am just putting in 8-10 sheets of notbook paper and will add as needed there...it will be cheaper and hopefully not as frustrating.
Try teachernet.com/how-to/manage/otter.html for a look at that notebook.
