In our room, we too have the board with the individual pockets and names. We have color coded slips of paper under the pockets, white (1st warning), yellow (5 min. time out), pink (15 min. time out), and blue (call home).
When a student misbehaves, we tell them to go get a D-Slip. They then go get the appropriate color and fill it out (name, date, offence, and how they will change their behavior). It is very rare that a student will receive a pink slip, and I have never handed out a blue. The one drawback I found last year was that my students were very attention seeking, and the white D-Slip just gave
them additional attention (getting out of their seat, filling out the slip, and returning the slip to the chart). Next year students names will go on the board and they will fill them out during recess!!
A perk of these slips is that we pulled them daily, and put them in students files for parents to look at during conferences!
For positive behavior, we had 'dinky dollars' which studnets would receive for good behavior, good grades, etc. They were able to redemm these dollars for privledges within the classroom during the year, and were charged for D-Slips, bathrooms usage etc. We also held 3 auctions where they could spend their remaining dinky dollars.
